It is probably the most-used mercury detox protocol today and has benefited many. Bex (a dear friend of our forum) has contributed a detailed yet easy-to-read article describing the Cutler Protocol to make it easy for people to begin using it. This article is posted below. If you have questions or comments about the protocol, please feel free to post them in our "Mercury/Amalgam" forum. The Cutler Protocol: How It Worksby BexStart chelation 4 days after ALL amalgams have been removed/replaced. The first four days is necessary to wait in case there is any swallowed bits of amalgam. This should be allowed to pass through the body first before starting any chelation. Start by using a chelator such as DMSA (or oral DMPS if one can get it). If using DMSA, begin on a low dosage first to check for tolerance/side effects. 12.5 mg is a good dose to start. One can increase dosage later based upon tolerance or lessen it if necessary. One may require splitting dosages if one cannot get such low dose capsules. The protocol is the most important part of the programme. The DMSA must be taken every 4 hours around the clock (yes, not just during the day, but throughout the night also). So alarm clocks are essential. This 4 hourly dosing must be kept up for at least 3 days at a time (or up to a week if time/tolerance allows). Then, take a break for at least the same amount of time spent on the round. E.g. 3 days on, 3 days (or more) off. Repeat this cycle for approximately 3 months. Changes to dosages must not be done during a round. If one finds the dose is too high and symptoms are intolerable or too difficult, STOP the round and wait for a few days or so, then try again on a lower dose. Some who are fast metabolisers may need to take the chelator every 3 hours instead of 4 hours, as they use it up faster than others! Oral DMPS is different in that it can be taken every 6-8 hours around the clock. It maybe better tolerated and more convenient, but not so easy to get hold of. May also be more expensive. If one does not wish to use these chelators, or they do not tolerate them at all, then one does not have to use them. Such a person can “wait it out” during those 3 months as the body will gradually reduce the blood levels over that time. But the chelators do help. After the first 3 months (following amalgam removal) are up, one can bring onboard alpha lipoic acid (the brain/organ chelator). It should now be a lot safer to use, once the blood/body levels of mercury have had time to reduce over the first few months. This brain chelating agent can now pass through the body and into the brain without bringing more mercury with it on the way (from the blood stream). Thus, it will start to chelate the mercury already in the brain/organs out of the body. Alpha Lipoic Acid (ALA) Chelation ProtocolALA can be used alone, or with DMSA. DMSA is optional. ALA is essential. However, DMSA may help with side effects and will likely make the chelation more effective. However, it is not absolutely necessary if one cannot afford it, or cannot tolerate it. ALA will still do the job on its own. ALA chelation protocol is similar to DMSA, except it must be taken even more often. Every 3 hours around the clock instead of 4. One maybe able to extend it to every 4 hours during the night if it helps with sleep, but should go back to every 3 hours during the day. Dosage should start very low. 12.5 mg, or lower to start. If using with DMSA, then both should be taken together – every 3 hours. This should last for at least 3 days, or longer if one wishes. Then, as above, take a break for the same amount of time or longer as a rest period. Repeat this cycle. One maybe able to chelate every weekend or so if they are working and cannot do this during week days or for other reasons. Basically 3 days every week or so is adequate. This may need to be done for many months, maybe even years, depending on level of toxicity and progress. Improvements maybe noted over time, but should not be prematurely accepted as “recovery”. There may come a time when the chelation is “keeping” a person “feeling” well as it is grabbing the mercury continually before it can build back up. This is why it is a good idea to continue chelating for awhile even after feeling well, to make sure that one is truly well! Maybe 6 months or so. Dosages can be increased up to 200 mg at the most (eventually). But most may do fine on much lower dosages and one should remember that it is not a race. Even very small dosages may prove difficult for many people who maybe more poisoned and sensitive than others! So better to stick with a lower dose that one can tolerate and can therefore do regular rounds, than dosing too high, too soon and then feeling extremely sick and unable to do regular chelation rounds. Only increase the dose later when one is certain that the present dose is well tolerated and has been tried for a few rounds already. Don't increase too much, too soon. Tread carefully. The protocol is designed in such a way as to reduce the redistribution of mercury during chelation. Redistribution is where a mobilising agent or a strong chelator is used inappropriately (randomly) and causes the mercury to shift randomly around the body and sometimes into worse and more sensitive areas (brain being one). This increase the risk for damage! This happens when the blood levels of the chelator are not kept constant and even and is an unnecessary extra risk when here is a protocol designed to reduce that! After 4 hours, the blood levels of DMSA will reduce. This is why it is necessary to keep taking it every 4 hours to keep a steady level of DMSA in the blood stream, which means a steadier movement and removal of mercury. ALA the same, except it needs to be taken every 3 hours to keep a steady level in the blood stream. This is why it is also necessary to keep the same dosage during the chelation round, as changing doses during that time may also cause a sudden rapid change of the levels of chelator in the blood stream and therefore the movement of mercury also. Again, if the person is finding the dose too high, one may have to simply stop the round and rest and try again in a few days on a lower dose (or take the chelator even more often than specified if one is metabolising it too quickly). Supplements and diet are necessary to help support the body's detoxification and health/healing. A sugar free, wheat/yeast/gluten free diet maybe needed. Perhaps even a reduction in all high carbohydrate foods maybe helpful if one has a serious candida problem, which is common in mercury poisoning. Wheat/gluten intolerance is also common. Addressing these alone can make a big difference to a toxic person. Some supplements recommended by Cutler are vitamin C, vitamin E, selenium (Note: selenomethione appears to be the better tolerated form according to Cutler. Take 2 hours away from vitamin C, as apparently vitamin C can interfere with it's effect/absorption according to some reports. If not tolerated, you don't have to take it), B complex, milk thistle, essential fatty acids, zinc, magnesium etc. Note: C, E, B complex, milk thistle, need to be taken at least 3-4 times daily, as they are quickly used up by the body, especially during the stress of chelation and being mercury toxic. ### Useful LinksI have provided these links for your convenience. The side effects of taking alpha lipoic acid(for those who are mercury toxic) without DMSA are so much greater than when taking ALA with DMSA.When I first started chelating I used ALA only and crashed hard after a few weeks. I tried chelating again with just ALA a few weeks later, and crashed again. A few months later I tried chelating again using ALA plus DMSA, and have been chelating since then, and have been able to increase the dosages. I started with 25 mg ALA + 25 mg DMSA. My advice for those starting out is to have their amalgams replaced first, then start with DMSA only, perhaps 25 mg every 4 hours. You could start this a few weeks after amalgam replacement. After a month or so you could add ALA at 12 and half mg, taking the ALA plus DMSA every 3 hours for 3 days(2 nights) around the clock, then take 4 days off and keep repeating the cycle.Some people can tolerate longer rounds, however for me the sleep disruption limits my rounds to 3 days(2 nights). I start a round in the morning, as soon as I wake up.
Some people get a very strong reaction to ALA, although this will be much less if DMSA is taken with it. Starting the ALA at a relatively low dosage seems like a good idea, especially if one uses ALA without DMSA.
You will know its time to increase the dosages when the side effects are very slight. If the side effects are too great though, then stop the round, and next round go back to the lower dosage you were able to tolerate. If you want to make progress, you need to keep raising the dosages(not too quickly though, perhaps every few weeks) until you eventually get to a high dosage(perhaps for some that might mean 200-300 mg ALA plus up to 250 mg? DMSA(some advise using up to 30 mg/kg per day of DMSA, which might be 250 mg per dose or higher for some, although others advise a target of 10 mg/kg/day. Imo 30mg/kg/day sounds like too much, and 15-20 mg/kg/day might be a more reasonable dosage maximum for DMSA. Some people get stuck at low dosages(25 mg or less of each) for many months and don't see that much progress.
After 5 months of almost steady chelation(I did take a few weeks off inbetween), I have gone from 25mg ALA + 25 mg DMSA to 150 mg ALA + 50 mg DMSA every 3 hours during each 3 day round. Generally it is advised to stay somewhere between 1 to and 2 to 1 ratios of ALA to DMSA, and I have just recently gone beyond that to 3 to 1. I don't plan to exceed 3 to 1 though, so my next increase will probably be to my DMSA dosage. Since DMSA is around 10 times the price of ALA, I have beeen trying to keep the ALA to DMSA ratio on the higher side, although I don't want it too high, as the side effects may bee too great then.

There is no necessary or fixed or recommended ratio of ALA to the accessory chelator (DMSA or DMPS) if you choose to use it. Dosage is flexible, timing is NOT. The more up to date discussion of how to chelate, presented far more lucidly, is in the back part (at about page 235) of Hair Test Interpretation: Finding Hidden Toxicities (which you can buy on this site). I strongly suggest being light handed in dosing if you are deciding for someone else, like your autistic child, because the side effects can be utterly miserable but so subjective an observver would be unaware of them. If you are dosing yourself, use whatever amount works for you and don't worry about whether it follows the numbers in my books. Andy
